It might be that some character in the file name is not getting parsed correctly by either the Roku or when handed back to the ReadyMedia server. Try renaming the file to something simple like x.mkv You may also want to confirm you are using version 1.2.1 and also confirm that the ReadyMedia service...

I wouldn't considered just adding an always on fan to indicate a design flaw. However, there appears to be no vents on the top of the Roku 4 so my guess is that the fan is designed to blow out the side of an unit. Given the Roku 4 is a little over 20mm in height, then it might the fan is less than 2...
